One ballot per person; must be 16 or older. Ballots must be signed and include name, address and telephone number.
Nominations must be made in 50% of the categories to be considered for votes. Copies of ballots will not be accepted. Ballots will be validated and tabulated by the Charles T. Mitchell Co. accounting firm.
